Before you begin

Please read through the What to Expect section and our Privacy Policy before completing the intake online.  This is where we provide important information about our program and the services we provide, as well as how we maintain your privacy and confidentiality. 


How it works

 

You can request a copy of an intake form by doing one of the following*: 

  1. Go to the homepage for FRC’s online administrative platform – Owl Practice . Click on CREATE ACCOUNT and enter your personal information.  This will notify the FRC Coordinator and will add you to our list of potential clients. The FRC Coordinator will then email you with to verify the type of service you are requesting (adult individual, child/youth, couples, family) so they can send you a personalized link to the  appropriate intake form. Once you have filled out the form and submitted it the FRC Coordinator will contact you with further information.
  2. Complete the REQUEST AN INTAKE form below. The FRC Coordinator will enter you into our online administrative platform - Owl Practice – and send you an email with a personalized link to the intake form you have requested. Once you have filled out the form and submitted it the FRC Coordinator will contact you with further information. 


*NOTE: You will need an email address to request a copy of the online intake form. If you do not have one, please call the FRC Coordinator to schedule a time to complete the intake over the phone.

Types of intake forms

 Our intake forms are tailored for the type of services you are looking for: 


Individual Adult Counselling

These services are always available through FRC. 


Individual Child/Youth Counselling (18 years of age and younger)

These services are available in January of each academic year (January-July each year). Please note that we require both parents and/or legal guardians to sign the informed consent for counselling services provided to individuals 18 years and younger.  If a parent or guardian has full custody, one signature will suffice.  


Couples Counselling

These services do require students who have additional training and experience in this area. They are generally only available later in the academic year or with intern students when available.


Family Counselling

When parents wish to attend counselling with their children this is called ‘family counselling’. This differs from individual child/youth counselling. These services do require students who have additional training and experience in this area. They are generally only available later in the academic year or with intern students when available.

What happens to my intake form?

  • Once you submit your intake, you will receive an email confirmation. 
  • Within a few days, the FRC Coordinator will notify you of the waitlist and provide details regarding the next steps.
  • Your intake form will then be placed on the waitlist.
  • When your intake reaches the top of the waitlist, a counsellor will be given your form.
  • Your intake form is securely stored with the FRC Coordinator until your counselling commences.  If you have questions, please contact the FRC Coordinator.


 **It is advisable to submit your online intake in a private, secure place.  Using a Public Wi-Fi Network does not protect your privacy and confidentiality.  Choose to do so at your own risk. **
